Mobile Health Consumer Terms of Service

Welcome and thank you for your interest in the Mobile Health Consumer website ("Website") and mobile applications ("Apps"). The Website and Apps and all content, services and/or products available on or through the Website and Apps (collectively, the "Mobile Health Consumer Services") are owned, operated, and brought to you by Mobile Health Consumer, Inc. ("Mobile Health Consumer" or "We"). Mobile Health Consumer Services are offered to you or the entity you represent ("You" or "you") subject to your acceptance without modification of all of the terms and conditions contained in these Terms of Service.

Mobile Health Consumer Services are targeted towards employees interested in obtaining consumer health information, participating in wellness programs, and managing their health plan-related expenses.

2. Eligibility

  1. ELIGIBILITY. MOBILE HEALTH CONSUMER SERVICES ARE AVAILABLE ONLY TO USERS WHO ARE AT LEAST 18 YEARS OLD AND LIVE IN THE UNITED STATES.
  2. Minors. You may access and use Mobile Health Consumer Services on behalf of a minor under the age of 18, provided that you are a parent or legal guardian of such minor.
  3. Use of Mobile Health Consumer Services. Subject to your compliance with these Terms, Mobile Health Consumer grants you permission to use Mobile Health Consumer Services as set forth in the Terms.

Mobile Health Consumer Services are intended for informational purposes only and are not a substitute for professional medical advice. Always seek the advice of a qualified health provider.

16. Interactions with Healthcare Providers

  6. Headings. The headings are for convenience only.
  7. Entire Agreement. The Terms constitute the entire agreement between you and Mobile Health Consumer.
  8. Claims. Any cause of action must commence within one (1) year.
